Presents findings of a review of the circumstances of the death of Mavis Flanders, an Aboriginal single mother who had been involved with various British Columbia social services and who died of a drug overdose in March 1997. Issues examined include the mother's drug use, its impact, and its treatment in this case; risk assessment and case management in child protection services; the management of the two apprehensions of Mavis's son; the appropriateness of the return of the son after the second apprehension; the adequacy of the supervision order of late 1996; the monitoring of Mavis and her son after expiry of the supervision order; case transfers between social workers and offices; social worker workload, training, and experience; the adequacy of social programs; and the role of family, neighbours, and community in this case. Concludes with recommendations addressing the issues raised.
