Improving Follow-up Care for Homeless Populations in North County San Diego

Homeless individuals face many challenges to follow-up care, such as stigma, mental health, mobility issues, and lack of resources. The purpose of this study is to analyze the current climate around follow-up care for homeless populations and propose a theoretical educational curriculum for healthcare workers based on the findings. Two Key informants were recruited through personal contacts, experts with the homeless population in North County San Diego. Three themes were identified through the participant interviews, which included: Stigma and bias playing a key role in follow-up care, no way of tracking homeless individuals' whereabouts, and the need for research. A curriculum focused on reducing stigmas surrounding homeless individuals was designed for healthcare workers. This curriculum was created to promote rapport between homeless individuals and healthcare workers. With the themes identified, the researchers were able to further understand how to educate healthcare workers on the importance of providing follow-up care for homeless individuals.
