Clinical Field Trials on the Dot Enzyme Immunosorbent Assay (EIA) for the Diagnosis of Typhoid Fever

Using the EIA dotblot developed in our laboratory, an initial field trial was carried out in 1988 using specimens from the General Hospital Kuala Lumpur, Malaysia. This initial study showed that the EIA dotblot was suitable for the rapid diagnosis of typhoid fever. Further evaluation of the kit was carried using specimens from outbreaks in Ipoh, Perak and Kuala Trengganu, Trengganu states. Results from these two studies showed that there was 100% correlation between S. typhi blood culture positive and EIA positive cases. The results were inconclusive for blood culture negative patients due to poor clinical and laboratory coordination. A controlled study was recently completed in Kota Bharu, Kelantan state using sera from 109 patients. Results of this third study again shown 100% correlation between S. typhi culture positives and EIS positives. Other results are presented and discussed in relation to culture and Widal results.
